This Menopause score card will help you get a clear understanding of the various symptoms that can occur during the menopausal transition. Please take a few moments to complete the menopause symptom score card, where you'll rate the severity of each symptom.
For each symptom, assign a score based on how much it impacts you:
* 0 = None
* 1 = Mild
* 2 = Moderate
* 3 = Severe
Once you've filled it out, bring your score to our first group session. This will help us tailor the discussions to address the challenges you’re facing and get you the support you need.
Instructions:
Carefully evaluate each symptom listed and consider how much it impacts your daily life. Use the scale provided to rate the severity of each symptom:
0 means you don't experience the symptom at all (None),
1 means the symptom is present but hardly noticeable or bothersome (Mild),
2 means the symptom is noticeable and has a moderate impact on your day-to-day activities (Moderate), and
3 means the symptom is very intense and significantly disrupts your life (Severe).
Be honest with your responses—this will help provide an accurate picture of your experience and guide better support and solutions for your needs.
